The central conflict begins at the point in the story when Greg decides that to make Rachel feel better, Greg painfully illustrates how he took for granted the opportunities that were denied to his closest companions. Eventually Greg finishes the movie and gives it to Rachel in the hospital as she lays dying. As a self-loathing narrator, he and Earl will make a movie for her because she loves their films so much.
